The recent Timor Sea (November 2009) blowout demonstrates that the industry seems to have learned little with regards to attitude toward risk and ensuing consequence. Indeed, type in ‘blowout’ on any search engine and you will see a catalogue of similar disasters stretching back 100 years – all with the same lame excuses as to why the incident happened.
